  • NEW BENEFIT ELIGIBLE EMPLOYEES

    Please submit your benefits paperwork within 30 days of your hire/eligibility date to complete your benefits enrollment. 

     

    BENEFIT CHANGES

    Changes to Benefit Plans Outside of Open Enrollment Require a "Qualifying Event"

    Examples:

    • Birth
    • Adoption
    • Marriage
    • Divorce
    • Loss of Benefit Coverage on Another Plan

    Benefit changes may be made within 30 days of the "Qualifying Event". Documentation of event is required to be turned in along with Enrollment Forms.

     

    DEPENDENT VERIFICATION

    Dependent verification is required when enrolling dependents on your benefits. Refer to the Dependent Verification Document List for acceptable documentation.
